Causes of Anterior Uveitis - Differential Diagnosis
Anterior Uveitis - 90% of cases of Uveitis
• Inflammation of middle layer of the eye
• Anterior-iris +/-ciliary body, Iritis=anterior uveitis
• Posterior-choroid
• Panuveitis: anterior chamber, vitreous humor, and retina or choroid
Unilateral
• Inflammatory: HLA-B27 related - isolated or 2ndary to spondylarthritis, Posner-Schlossman Syndrome
• Infectious: HSV and toxoplasmosis (common), VZV, syphilis, TB, post-strep, Immunocompromised-CMV
• Chronic Inflammatory: Sarcoid, JIA
Bilateral:
• Inflammatory: Behcet's, SLE, TINU syndrome (w/ renal involvement)
• Medication: Rifabutin
• Idiopathic: Unilateral/bilateral up to 30% of cases
• Mimics: Malignancy - Leukemia/Lymphoma
Pearl: Acute onset anterior uveitis in an elderly patient is likely HSV/VZV. In young patient/ consider HIA B27-related syndrome
