DPP-4 Inhibitors - Pharmacology Summary
Mechanisms of Action: Inhibits DPP-4 (dipeptidyl peptidase 4), which normally inactivates GLP-1 and GIP.
Examples:
 • Sitagliptin (Januvia) - Once/day tablet
 • Saxagliptin (Onglyza) - Once/day tablet
 • Linagliptin (Tradjenta) - Once/day tablet
Major Advantages: None specific
Contraindications:
 • Recommended to avoid combination therapy with GLP-I receptor agonist
 • Personal history of pancreatitis
 • Saxagliptin is contraindicated in heart failure
Common Side Effects and Toxicities:
 • Joint pains, myalgias
 • Association with pancreatitis (though not definitively cause/effect)
 • Saxagliptin is associated with t risk of heart failure hospitalizations
