Focal Takotsubo Cardiomyopathy on Echocardiogram
70s F with acute dyspnoea, no pain. POCUS in ER- What's your pick of the culprit(s), if any?!
There is focal akinesis, without thinning, of the mid anteroseptal and mid posterior segments. Not in keeping with a single coronary distribution, nor typical of ischaemic insult given preserved downstream function. Could be variant takotsubo or myocarditis. Still needs angio! Angio demonstrated patent coronaries!
