Pulmonary Embolism with Clot-in-Transit on POCUS Echocardiogram (A4C)

RA and RV are dilated. Multiple hyperechoic clots seen bouncing around in the RA. RV free wall akinetic with spared active apex- McConnell sign. Suggestive of hemodynamically significant PE.
