Non-Infective Endocarditis - Pathophysiology
Non-infective Endocarditis Pathophysiology:
 • Valves usually undamaged - Etiology poorly understood but thought to be combination of different interacting mechanisms
 • Circulating immune complexes +  Carcinomatosis → Hypercoagulable state → Valve destruction or failure is rare
Infective Endocarditis Pathophysiology:
 • Underlying damage to valve endothelium is common →  Bacteria adhere to damaged endothelium and propagate → Vegetation forms and promotes activation of clotting cascade → Fibrin and platelet deposition over clusters of bacteria → Valve destruction and failure
