Major Clinical Trials and Evidence Summary for Transfusion Thresholds in Critical Care

TRICC - NEJM 1999 - ICU
FOCUS - NEJM 2011 - Hip Surgery
Villaneueva - NEJM 2013 - UGIB
TRISS - NEJM 2014 - Sepsis
TRICS3 - NEJM 2018 - Cardiac Surgery

Transfusing blood is a liquid transplant.
Blood saves lives but multiple studies show that PRBC transfusions may be unnecessary and cause harm.
Unnecessary transfusions have been observed in critically ill, septic, surgical, GIB, and cardiac surgery patients.
Transfusions should be administered in the context of improved oxygen delivery.
ALI is the deadliest complication of transfusions.
Transfusions are immunosuppressants.
