RV Strain in Pulmonary Embolism on POCUS Echocardiogram

A patient is brought by EMS intubated and hypotensive. What is your next step in management?
Resolution: Alteplase. Significant RV strain from massive embolus-->LV inability to pump-->hypotension/peri-arrest. In the setting of hypotension & this echo (assuming pt full code, this pt was 60yrs old), give tpa (alteplase) 10mg bolus followed by 90mg iv infusion over 2 hours.
