Enteral Feed Formula Selection Guide

A basic guide on how to choose which enteral feed formulations and brands are most appropriate to select depending on the clinical situation.

Most Wards Patients:
- Formulation: Standard Polymeric
- Common Brands: Jevity 1.2, Osmolite 1.0

Diabetes:
- Formulation: ↓Glycemic
- Common Brands: Glucerna 1.5, Glytrol 1.0
 
CHF:
- Formulation: ↑Calorie, ↑Protein, ↓Volume, ↓Na
- Common Brands: Osmolite 1.5, TwoCal HN 2.0
- Additional Notes: Osmolar load can cause diarrhea

ESRD on HD:
- Formulation: ↑Calorie, ↓Volume, ↓K, ↓Phos, moderate Protein
- Common Brands: Nepro 1.8, Novasource Renal 2.0
 
CKD no HD:
- Formulation: ↑Calorie, ↓Protein
- Common Brands: Suplena 1.8, RenalCal 2.0
 
Short-gut, pancreatitis, severe PCM:
- Formulation: Peptide / elemental
- Common Brands: Peptamen 1.2/1.5, Vital AF 1.2
- Additional Notes: High Osmolar, start slow

COPD, Vent weaning:
- Formulation: ↓CHO, ↑fat to lower CO₂
- Common Brands: Pulmocare 1.5
- Additional Notes: Avoid in pancreatitis, fat malabsorption
