JNC8 blood pressure goals for the treatment of hypertension are a lot simpler than they may
JNC8 blood pressure goals for the treatment of hypertension are a lot simpler than they may first seem! The first diagram shows the algorithm that JNC8 provides. When you look at the second diagram, you realize how simple it is! If your patient is 60 years or older and does NOT have diabetes or chronic kidney disease, then the BP goal is <150/90 mmHg. All other patients have a BP goal of <140/90 mmHg. There are newer recommendations from the 2017 ACC/AHA guidelines, but those remain controversial and some organizations have chosen not to endorse them. Pharmaceutical Management of Hyperthyroidism / Thyroid Storm Beta-Blockers • Decreases sympathetic hyperactivity • Partially blocks peripheral conversion of T4 to T3 (propranolol) Propylthiouracil (PTU) • Blocks synthesis of thyroid hormone • Decreases conversion of T4 to T3 • Faster onset of action than methimazole • Preferred in pregnancy Methimazole • Blocks synthesis of thyroid hormone • Teratogenic Iodine • Inhibits release of stored thyroid hormone • Administer only after synthetic pathway has been blocked by PTU or methimazole (approx. 1 hr) Steroids • Decrease peripheral conversion ofT4 to T3 • May treat autoimmune process in Graves' disease • Treats adrenal insufficiency that may accompany storm #Hyperthyroidism #ThyroidStorm #Management #Pharmacology #Medications #Drugs

Group A Strep Pharyngitis, also known as Strep Throat. In addition to symptomatic presentation, a rapid antigen detection test can be done at point-of-care with a throat swab. This is a great expanding area for pharmacists in the retail/community setting too! If positive, no culture is needed, and patients should be prescribed one of the treatment options recommended by IDSA. That's because this test is highly SPECIFIC (see older post regarding specificity vs sensitivity for a helpful stats review). If the throat swab is negative in children or adolescents, it's recommended to have a follow-up culture to be certain. ---- There are options for patients with penicillin allergies as well, but let's be sure these patients actually have a true penicillin allergy! Large majority of patients with "penicillin allergy" listed in their chart do not actually have a true allergy. This is another great intervention for pharmacists - penicillin allergy testing!
